Look for Meds That Can Come Off Most Med Lists

You'll see continuing efforts toward deprescribing...since polypharmacy is the number one predictor of adverse drug events.

But whittling down the med list isn't easy. Use a "less is more" approach...to possibly reduce med-related admissions, falls, and death.

Identify meds that may be causing side effects. For example, review med lists for patients who've had a fall...and look for opioids, benzos, antipsychotics, etc. These may have been needed before, but now risks may outweigh benefits. Stop, switch, or lower doses when possible.
