One in four heart failure patients are readmitted within 30 days of hospital discharge...which isn't good for patients or reimbursement.

But early follow-up helps. It prevents one ED visit for every 9 patients managed...and one readmission for every 52 heart failure patients.

Think of early follow-up as very clinically important...it makes as big of a difference as using a statin post-MI.
