Pepcid is excreted renally. A lower dose is already recommended for SEVERE renal insufficiency...creatinine clearance < 10 mL/min.
Now Merck also recommends lowering the dose for patients with MODERATE renal insufficiency...creatinine clearance < 50 mL/min.
