Updated guidelines no longer recommend acetaminophen at all...or tricyclics (amitriptyline, etc) for back pain that lasts more than 12 weeks. Recent evidence suggests patients don't benefit.

For acute back pain, continue to promote nondrug strategies in the ED...such as massage or heating pads. Reinforce that nondrug measures can help...and that acute back pain usually improves in 2 to 4 weeks.
